A follow swap is when someone follows you and you follow them back, swapping follows.

Sometimes it's to increase engagement/get themselves out there but essentially exchanging follows to artificially inflate your follower count, which is often considered a manipulative tactic and can be frowned upon by many users and platforms.
The main issue with follow swapping is that it doesn't necessarily translate to real engagement with your content, as people who follow you solely to get a follow back might not actively interact with your posts.

So in this instance he's insulting her by saying that she only has followers because she follow swaps but doesn't have any real followers
