In the past some users have actually gotten a title from unique situations. Here's some examples:

• @Clipnails got a 'Bocchi-chan lover' title back when V5 was first starting
• @whosaint got his 'Towa-sama's Lover' title from his obsession thread of Towa
• @ReGats got his title entirely removed after complaining as a newbie

Again, these are very special situations though, I don't think admins would do this kind of stuff nowadays.
