Something traumatic I feel definitely happened in her past she's kind of lying to herself and accepting this happy go lucky side without building a relationship with them, hopefully it can be resolved

Based on how things are going, and how they seem to be getting weird, my guess: She slipped through worlds and met his grandmother who took care of her, sending her back, and she knew the REAL him as they played as kids.

Throwing this out there because I'm getting vibes from an author who is really struggling to find a story here in a land of well-trodden tropes, reaching for the lesser used ones to find something to grab onto while also trying to get creative/original.
