I don't think a hard on is necessary for it unless you want to go generic cuck and he literally picked her for that role so at the very least he seems pretty willing to put himself in front of Sakura's sexual behavior as a third party. It's seems pretty cuck coded to me.

It looks that way because Arir is bad at conveying what she wants to show it's shionbu suffering that she is showing not him being a cuck.
