It's not very common, normally they are all very honest about it. If a guy is using a girl's avatar, they are honest about them being a guy, in case the voice didn't make it obvious. Their audience doesn't care. Parts of the audience even prefer it that way.

Bringing back elements already established in the story, and using them to solve problems rather than making up something in the moment to solve them, is normally considered very good writing.

And we reached 100!!! This is why stories that end with the Characters entering into the relationship are wasting a lot of potential. This is the kind of thing that can happen when you have them already in the relationship!
