The signboard is in Japanese. Is Japanese writing used by everyone in that world, or was it a universal language?

None of the people he came with could read any(?) of it so my guess is no. It’s unrealistic that he could read it anyway since he’s the first person reincarnated in that world. Even if he wasn’t, it would probably have originated from a different era and evolved into something totally unintelligible to a modern Japanese person.

Why don't these Isekai Protagonists ever reincarnate in these areas instead of Medieval Europe???
The Medieval Period spans a thousand years, between the 5th and the 15th century. Based on the buildings they use in some mangas they are in the enlightenment era (the late 17th and 18th centuries) but they have yet to basically invent anything. It's just weird and all over the place tbh.

Everyone's always making such a big deal about mixed bathing as if it didn't exist anywhere else. Like in the standard European Medieval setting.

"I thought you were talking about food..."
Always gotta be the one.

And modern magical equipment. Though it's not like plumbing didn't exist since long before that.

Yeah, it's always so schizophrenic. Aesthetics are usually post-Medieval, especially clothing. Food development is often non-existent and tastes like crap, as if real people would actually not do anything about that. Weapons are anything pre-Industrial, except that swords are often considered blunt, which was never a thing. Modesty is almost always Victorian, which was the peak of it, whereas in most of history it was more lax than it is today. I think the worst thing I've seen an isekai character "invent" was a spinning top toy. Those are prehistoric.

One of the great mysteries of the Japanese fantasy: why is most of it located in historical fantasy Europe looking place, not historical fantasy Japan? There are, naturally, many such series as well (such as Inuyasha, known by everyone), but they are still a minority. Yet most of the isekai fantasy located in Europe inspired sword and sorcery will have the Japanese MC miss Japanese/East Asian things and work hard to acquire some of them. Instead he/she could have been thrown into historical fantasy Japan and not miss a thing, instead feeling right at home (within reason, it'd still be a fantasy land). There's certainly a weird contradiction here.
