Someone with more background needs to fill me in on this one.

I knew US torpedos in WW2 initially weren't all that reliable. Contact/impact fuse issues I take it?

Edit:
Wikipedia crawl netted me:
  • Running deeper than set
  • Magnetic trigger causes premature detonation
  • Contact exploder generally didn't work
  • Not gyroscopically stable, resulting in a nonzero chance to hit the ship that launches it
Whoo boy. At least they fixed it later lol

1943. They spent more than half the war with torpedoes that didn't torpedo correctly.

For bonus points, the Bureau of Ordinance (the internal bureaucracy of the US Navy at the time) both refused to do much testing internally, and like the current Sig-Sauer snafu, were, "pathologically incapable of admitting to a mistake". When even the best submarine captains were watching Mark 14's slam into enemy ships and do nothing and then complaining, the Bureau's response was "must be your fault".
