Re-read chapters 43,44, and 45. They're all aware that he is definitely fucking other women. To add on Sakura brought Misaki over for him to fuck and he also would have told his family where he was when he was fucking the guards in the hotel and also they would have seen the sperm centre staff go into the room with him and come out with filled condoms. It's probably just the difference in knowing he's doing it and seeing it happening.
