You two are overthinking this. If either happened it's unlikely he would ever be able to picture Hayato as anything but a girl. Especially the latter - once you go mom, you don't go back.

i think it's a legitimate question, and one that Keiichi and Hayato would definitely consider if they ever started dating or got married.

we know that Hayato's gender is determined by the consensus perception of people in his vicinity. in the scenario where Hayato did become pregnant as a girl, it would be safer for him to remain in isolation to avoid accidentally turning back into a boy. and there are actually realistic cases where this could happen. Hayato appeared as a girl to Keiichi before Keiichi even saw him, meaning that someone who had not seen Hayato as a pregnant female may be in Hayato's vicinity and imagine Hayato as they knew him (a boy), thus turning Hayato from a girl to a boy. regardless, it would always be ideal to keep a pregnant Hayato isolated from anyone who even knows him (bar Keiichi maybe) to avoid any accidental subconcsious genderswaps.

as for what would happen if a pregnant Hayato were to turn back into a man, we can only make inferences based on what's already established in the story. determining which specific aspects of Hayato change from a genderswap can help our speculation. one thing that a genderswap doesn't change is Hayato's clothes. this is confirmed on the train when Keiichi realizes that Hayato isn't wearing a bra in girl mode (of course he wouldn't be, as he traveled there in boy mode). however, Hayato's hair length changes completely when a genderswap occurs (short hair as male and long hair as female), meaning that a genderswap changes more than just his body's physical structure and reproductive organs; it also transforms something that can be readily modified (like hair or nail length).
