NGL I am super sus about the fact that no one has clarified whats become of his sister and grandparents while in the coma, we've only seen his parents show up at the hospital for him. Did they die in the accident and I forgot them mentioning it or has that just not been established yet?

She's been gaslighting him so hard that his own mental state involves gaslighting himself into finding a means to blame and hate himself more so instead of her. After all, this "conversation" with his cousin and then his Mother...
