The "friend" didn't open the door. The father stood really close to the bars, so the "friend" could grab the knife through the bars and then hold the fathers legs before slashing them. The person in the door frame is another guard, not the escaping "friend".

As far as I understood it, the stoy is told in two time frames in parallel. The small "child" character is the earlier time and as maxibon98 said, it somehow transforms into the silicon creature of the second, later, time frame which shows the now transformed character walking throught the...

I don't really get the meaning of this work. The other chapters were already pretty symbolic, but this one is now finally over my head. Clearest explanation I can think of is attempted suicide and in the limbo between life and death the other girl and her mother show her that life is more...
