From what I remember in part 1 the door was a representation of denji surpressing the memory of him killing his dad. So this was stated near the end of part 1 I believe. Edit: This is basically Asa's door moment.

Death is revealed to be what was thought to be Fami. She then controls real fami and Fakesaw man with the falling Devil to fight Yoru and Denji. The fire devil shows up and tells Denji off. Yoru gets a power up middle of the battle and turns falling into a cape and claims she loves america...

Might have came back due to how fire is still around thus it's easy to make flamethrowers again in the modern era. Like how nukes came back eventually. That or he wasn't really eaten.

God that panel with her disappearing has such a sad feeling to it. The art is as incredible as always.
