LN explains it better. There are two reasons why society would look down on Filimina for having a scar on her back. The first is in society's eyes that have an anachronistic view of women as suppose to be beautiful, have smooth skin, and appear perfect without flaw with the scar being a big flaw. Kind of like Prince Humperdink (for those who read the Princess Bride instead of just watching the movie,) who wanted a wife whose beauty he could show off to make himself look good as her husband. The second reason is that the kingdom's culture and history is immersed with interacting, using, and being helped by spirits so having a scar inflicted by an enraged spirit makes her seem like a delinquent who angered the spirits that exist and help the kingdom. And as everybody knows people prefer gossip, intrigue, and having somebody to look down on over facts and circumstances. After all, they can act high and mighty by putting her down as someone who enraged the spirits as an undesirable person based on rumor and biased opinion which they couldn't do if they bothered to find out that it was the result of a childish mistake as a little kid not even ten years old. Also, some residual energy or something from the spirit that attacked her remains in the scar so no other spirit will have anything to do with her either. She can't summon or use spirits for anything magical. This means that any healing magic or attack magic that uses spirits even by somebody else will be refused to be cast on her by the spirits.
