There's the fact she says she doesn't want to talk about her husband, but then she says she actually loves the guy at the phone. Also the beginning of chapter 14, a lot more ominous now we know she wants to die, and the fact she kept wearing the ring.
My take is that she really loves the guy that gave her the ring and that guy died in the past; also, she suffer from some painful incurable disease. Then, either that guy is her late husband, and the guy at the phone her brother or something, or he was her true love, and the guy at the phone his actual husband.
