It's actually a really old and tired kdrama cliche for the MC to have some incurable, inoperable medical condition. I don't know if Higashimura is intentionally paying homage to that or if she's that cheesy herself, but I found it pretty humorous.

Thanks for that Chapters, ThePaulBunyanTrophy! I'm just here because I love this author's sense of humor. Shouko is pretty relate-able. I loved the flashbacks to the marriage interview meetings that never worked out, just because its something I know is true and I haven't seen it in a webtoon. I love the comment made by @Corgiattackkk!
