Essentially, he made a wish with that masked figure to be her boyfriend before she died.

Through some sort of time travel, he is both the little boy and the adult (?) boyfriend.

But somehow, he still can't save her, which is why people don't think it's any sort of happy ending.

You don't understand, the happy ending is after this Oneshot, when the man walks away the weird monster tries to take his life, but he instead yeets the monster through the hospital window, and goes on to find a cure to the girl's illness.
