From what I remember, people of certain origins, particularly East Asian ones, tend to have harder earwax than people from other parts of the world (like of European descent). So they have to clean the wax out to avoid issues and because it's harder to do and requires more care it has to be done by someone else and requires a certain amount of comfort and closeness.

It's basically meant to represent an extreme sign of trust and closeness that one character is willing to let another one do it for them.
