She's a hot corporate secretary and he's a hot doctor, seems plausible that he was like a token, display boyfriend to her rather than someone she felt a genuine connection to, if we are to interpret it generously. The manga doesn't explicitly say any of that, but it also doesn't say they had a great relationship, it just says they dated for two years and that's it. One scenario is as likely as the other.
