Ojiro aims for the liver because of the high blood flow to that location, it means you bleed out fast. Because that guy sold some of his liver, the blood vessels in that area have been closed off, so while it hurts, it isn't killing him.

It's, uh. It's a bit of a stretch, but the logic can be followed.
