The ending is basically the MC being transported to what could essentially be God and he is told: So you are the only survivor of this "war" that we had caused by sending the 3 transfer students to your time-line.
Therefore, they are offering the MC the chance to create a brand new time-line exactly had he wants it and he will be transported there to do what he wants, live out his life or his fantasy or even change it.

We get a glimpse at this new time-line in the very first chapter of this volume in chapters 37.
Of you read that chapter and remember everyone's faces and names then you can see that everyone is alive. Supposedly no one has powers, the MC is still a boy. The girl is confused because this new world feels dreamlike to her like she has memories of the original world.

Chapter 37 is basically the epilogue. The conflict started when the sister of Samurai bishojo from an explosive curry.

We even learn that a Transfer student ( a traveler from another timeline) tried to invade this timeline but the MC made it so Transfer students are not overpowered in this time-line like the original so he immediately got his butt stomped.

About the body parts that are necessary. I am not 100% sure about this but the reason that the MC was able to become a transfer student was become he changed his perspective about himself and he had an anchor of purity about him that made him special, someone loved by God. That part must not be spoiled in any way.

In the case of MC or Otome, because he was a transgender boy, he was able to protect his lower female parts from being spoiled by others during this war.

So, if there was a price to paid in body parts then the MC probably gave him his female body so that he can have his perfect Universe. This is why at the end he is a boy and maybe not a transfer student. He lost the thing that made him special.

When I reread chapter 36, I realized this wasn’t the first time the transfer students (Yukimi and Kurosuzu) had arrived in another world through a contract. So now I’m thinking: creating a new world seems to require a transfer student to travel to another world, fulfill a contract, and bring back a body part. If they end up disliking the world they create, the cycle just repeats again.

I'm pretty sure the MC is still a girl, she's got noticeable boobs. Looks kinda weird because the artist tends to do pointy broad shoulders and a narrow waist for both sexes.

I think the MC became a transfer student because he defeated the rape dude. The rape power was supposed to be absolute, but because the MC had a penis at the time the rape dude couldn't actually take his front virginity. Since MC defeated an absolute power, that meant he was favored by "God" and therefore a transfer student. He just hadn't gained the realization needed until that conversation.
