I'm not entirely sure how to count them given the differences of the knots there, but regardless of the amount it's supposed to be counted as, that guy's probably a bigshot from the Beggars Union. Could be an elder, the supreme elder, or even the leader of the whole damn thing? Probably mentioned in the next chapter or so after making such a point of showing the knots.
