I'm reading this as Sei has a morbid curiosity or fixation with seeing how his mother eventually passes on. This is a woman who's manipulated him his entire life and only had him to chase after some feeling of belonging which she never really achieved. She's done irreparable damage to him, and I think he is feeling the shift in roles where she is now the vulnerable one.

ah Seiichi became Seiko and Seiko became Seiichi. He has now become the mother who is distant and emotionally dead and distant but caring for the child in hopes of churning out any sort of happiness or reason for existing. It's all come full circle
