All her limbs gets amputated and three, the legs and one arm, get replaced. Her left eye also gets removed but not replaced and the curse on the other one is lifted. As for her "girly parts", despite the damage they're left as is but she's informed she cannot have sex or have children anymore.
Way later on, when they got in contact with the elf village and Reszure's family all her limbs, eye, "girly parts" and all other scars and damages get fully regenerated but this is on the final endgame of the original series.

I think it's more soul transfer than brain transfer. Transferring a brain would be far more difficult.I always figured that Adam put his brain into a loli. I am now unsure if that is the case now that the new lore has been dropped.
