Do you know where the temple is? It's not on the top of your head. Obviously it isn't good to be hit there but the crown of the head is a much sturdier place than the temple. Also, you probably won't get "crippled" by being hit to the temple unless it causes specific forms of brain damage. Now if the elbow had landed to the base of the neck, that would have risked crippling her.

The temple is the one between your ears and the eyes. People are more likely knocked out when hit in the lower jaw (close to the chin) than being hit in the temple. People will also be less likely crippled when hit in the temple or the crown than the nape or the back of the head.
