I've just known that Angel's heart is the continuous series of City Hunter? And apparently in that universe, Kaori dies and her heart got transplanted to the protagonist. Does anyone know if this is also true in City Hunter? I've seriously had a mental breakdown when I read this.

NO, Angels Heart isn't direct continuation of City Hunter, it's alternative universe with similar characters, some of them have different traits compared to City Hunter alter egos. Thank God Tsukasa stated it clearly that AH is alternative universe same as part 7 of jojo to previous parts from 1 to 6.
