I’m getting the feeling that Saito is misunderstanding Akane here, I don’t think her sister is actually dead, since she never said that. She only said that she was “far away” which could be taken metaphorically, but also literally. Only time will tell, but she doesn’t seem to be acting as if she’s dead.

Akane would probably be more honest about that with him, she’d probably even cling to him for support remembering it if that was the case.

So she's the sick girl I've been seeing in Akane's flashbacks for a while now. There's definitely some implication that she's no longer in the world of the living if we base it from her melancholic mood and the operating term had, but it's also dangerous to assume it is so.

Y'know what, I don't remember anything about her parents' current status. It could be that her parents separated and remarried, splitting custody. That could work for "I had a little sister."
