For a woman, being an "exclusive guard" doesn't mean being the only guard or woman around. It means the woman herself won't guard any other man but her own. Also, it's unlikely for a man to be with just one woman - usually there would be more than one, if the man is willing to have such relationships with women. Just read the 2-E girls thoughts, they naturally assume Sou will have several wives (including his mom and sisters) and while they want to monopolize him, they still accept the fact that hogging him is impossible.

So, how would being an exclusive guard work? Some ideas:

I think we can assume that men are not only forced to donate sperm on a regular basis. It's likely that they receive financial support as well. Let's take families with boys for example - the mothers are getting substancial monetary support from the government to help them raise their male offspring safely. Men are an invaluable resource so it should be safe to assume that even adults benefit from that to a certain degree. That world is weird, so they might even have official contracts for being an "exclusive guard" - like signing a contract together with him makes the woman a civil servant responsible for protecting that man. She gets paid for keeping him safe while the man receives his usual financial support.

Another option would be a family consisting of a man and several women, where one of them is a trained guard and mainly
responsible for safekeeping the male while the others females work normal jobs. You mentioned guards in school being three-person teams - if more than one woman is a trained escort, they can rotate that responsibility, but there might still be one among them being exclusively assigned to him.

The next option is probably more for rather rich families who can afford it. In that case the family hires a trustworthy woman exclusively to protect the boy/man - a dedicated guard wife essentially who's job would be to keep him safe all day and also make sure he produces an heir(ess)...
