According to a couple of googling results, ambulance trips are free in Japan, though the treatmeant aboard isn't. Treatment as a whole is covered for the major part by insurance (which is obligatory in Japan) and for the minor part by the patient, depending on their income level.
Consequently it's apparently a problem in Japan that people try to use an ambulance as a means of transportation to a hospital even if they were capable of travelling there on their own just fine. Reading about that, I wonder if it was a problem where I live as well, since apparently ambulance crews are quite eager to refuse transportation if a person seems capable of using a taxi, for example.
