So what I'm getting is that she did save him because she still loved him....BUT being a Japanese ghost and all, she's still somewhat malicious, so she also cursed him. The curse being that if he loses the note, he falls into madness/gets possessed by her, but as long as he keeps it as the keepsake he currently does then he'll be fine.
