I'm assuming that the voice was his own trauma child version and that he has a split personality to cope. He probably also doesn't really recognize his sister anymore.

The body isnt accurate but her psychological problems that body building and fitness is 100% accurate, in fact it could be that her psychological problems of body dysmopmrphia has caused to have formed this body of hers
