Sounds to me like she has Tourette Syndrome. It can manifest in many different forms but that sounds like classic Tourettes.

This reminds me a lot of the Clarissa web comic in how the family enforces a happy Christian funtimes act and the abused girl who breaks that image is abused even more for not complying. And the stuffed rabbit imaginary friend.
