"Ippei" seems to unambiguously be a guy's name, so I suppose that he has a dick and is contemplating on whether to use it.

Men also have mammary glands but normally don't have the prolactin levels to stimulate the production of breast milk.

Also, the mangaka's art is very stylized and I doubt they wouldn't draw a woman as flat as a board if they wanted to get across that such a character isn't well endowed.

I'm honestly curious as to the extent of their relationship. The author's 34 of his characters include these two, so it's possible that they have a physical relationship. He's done 34's of all the other relationships he's made for his series, after all. Then again that could just be 34 for the sake of 34. But otherwise I'm curious to what leads him to care about her and put forth effort for her. We'll probably find out, though given the sporadic release of this series, it may take a long time.
