It's a joke because otherwise you'd realize that a high schooler and college student are pegging a middle schooler. So they're all 18+ years old and not blood related (despite that being the punchline of the manga).

I mean, I was wondering the same. The note seems to say everyone is half siblings, but I'm under impression that Aoi is actually the one and only true siblings here though...

Nah, I don't remember her name either, but that ex-idol girl mentioned her name in either this or last chapter, so I just ran with it.
I'm no better than you in remembering the original story; that's exactly why I was second guessing when the note suggested they're all half siblings. Had I remembered clearly, I could have said the note is correct or not already.

My personal conjecture: since the fanbox content is basically porn, author changed this into a "spin-off". All characters were turned 18+ and became step-siblings (not blood-related siblings), probably to avoid some legal(?) issues. The original has three of them initially middle-schoolers, and all of them (save Aoi) are half-siblings (blood-related, same father different mothers).
