There are some small changes in my TL.
I finally got the last name of Riku in Furigana, so his last name is actually Yamabe, not Yamanabe.
Kanako (Riku's love) calls him Ritsu (nickname)

I'm pretty sure she calls him Rikkun in the raws. りく + くん = りっくん

I'm aware. It's just that from my understanding the つ is there to indicate gemination and not to imply that she calls him Ritsu. Like how 特権 (とっけん) is broken down into 特 (とく) + 権 (けん)

'Azeno-cho' as it is called in universe is in reality Kyotamba-cho.

Shunpei's middle school is this place. This angle looks close enough to what's shown in the panel.

The main bus stop is Hinokiyama bus stop; it looks exactly like it does in the manga and is a ten minute walk away from the school.

It seems to be the main stop of the area so like ten buses branch out from it - the exact bus Shunpei and An take is apparently Shitsumi line.
The Hachiman shrine in Chapter 12 is the giveaway, again the entrance looks exactly like it does in the manga.

Shunpei and An's places would be a bus stop or two up and down the shrine. An's stop is probably here, doesn't match exactly but does seem pretty accurate. Not entirely sure where Shunpei's stop would be for now.

With all of this details, I wouldn't be surprised if this is the mangaka's hometown

Whoah, it is sure quite far from Kyoto's city. Riding train will take around 2 hours, so 4 hours round trip. Not mentioning there's a chance you have to wait considerably for the train, or bus. Google Map shows a resort hotel called DOGVILLA at the center of Kyotamba. What a curious name.

If I'm not mistaken there's a site with map for otaku pilgrimage? If users can add their own destination, surely this can be added to that map.
