Uh, it is really Smut? I don't think it's the right word to use. It is more like Mature with Nudity in it.

The concept is interesting, but sort of a challenge for me.
I'm good at handle horror, yes, because they always not about violence. If they do, small content is enough.
But talk about transplantation organs, I'm not. Blood-related stuff, I'm always easy to imagine something and it is disgusting.
Even when I watch SAW, I can hardly follow the story because it is too sadist.

I don't know where the story will go. But I'm sure there must be up and down. 11 volumes is a safe number.
I expect something big from this manga. And it is too soon to judge.
