Dies 'over 10 years ago' at 5, lives 6 years to 11 en isekai, dies, lives 6 more years to 17, dies, back to isekai land 10 years after death at 11. So like 4 years unaccounted for?

Time flow in the two worlds isn't identical.

Not as hard as that but probably will come to that.
The first one probably just holding hand or just touching anywhere. But wonder if just touching will still be enough in the future.

Perhaps, but she came back the first time at 11 years old, not 13 and a half. Or time flows the same and their years have fewer days, or length of day is different. All easily handwaved if the author's inclined and if it was just a goof... but especially if it's different this time, I was kinda pointing to it in an "I wonder if that will be significant" way.
