It can be anything like a normal film with Shinobu's technigue to push Sakura to perform extremely well or it's an AV film or it's a private film Shinobu do for himself as he discovered his cuckold fetish.

FYI, AV director's income is lower than film director. Normal AV directors try hard to become a normal film director.

So, if Shinobu's been succesful as a normal film director, why would he accept the job as an AV film director? Because the AV actress is Sakura?

It's not real sex simply because Shinobu isn't an AV director, and the producer commissioned Shinobu to direct Sakura's scene because Fumi's simulated sex scene was a completely success.

What I got from the MTL is that Shinobu did a lot of movies with focus on sex scenes (but no AV) in his career, but stopped focusing on those when he got famous enough. He accepted the role as director in a movie that happens to contain a sex scene and accepted Sakura as the main actress.
