I know the author is foreshadowing her accidentally revealing her sexuality to her parents, but honestly, I don't think anyone would consider that hug romantic or sexual at a glance in the West. I mean, they're in a hospital for crying out loud and her brother is near death. Being comforted by a friend's hug is completely 100% normal in that situation.

While I agree that I a hug between friends is easily what this could seem like, it's also important to note that he's seen then interact while knowing he was there AND while they thought they weren't near him. It's very possible he got a romantic vibe, or at least the vibe of people who are closer than the average pair of friends. If he's a homophobe this might just give him more reason to be critical (unjustly)
