I red it somany years ago so please forgive me for being a bit vague.
School setting romance-ish with an interesting plot twist at the end
ML in high school gets a crush on a transfer student who looks like a girl he met as a child. He comes from a well off family and has a hereditary heart condition (one of which his father died of).
Blonde transfer student who's loves swimming and is in the swimming club she comes from a poor family single mom and her dad left when she was a kid.
They slowly fall in love but then clues leading to the twist. Basically they have matching keysthat go to a house in the country (think it was there paternal grandmother's house where I believe they met as children).
And they find out that the boys father cheated around the same time his mom conceived him. Father had a secret family and when he died left nothing to the girl and her mother but the key to the house. They find out they're half siblings and due to shock the boy succumbs to his condition.
Girl saves him though. Both are heart broken and she never wants to meet him again and transfers schools. THE END(as far as I can remember)
