Jesus Christ Japan, phimosis is when the foreskin is so tight that it either causes significant pain when retracted or it can't be retracted at all. It causes significant problems during sex and masturbation and wouldn't be commonly found on wank fiends like Nezumi. Phimosis is not the same as having your foreskin intact. For fuck's sake, that's normal. Why does every ecchi manga think phimosis is like that?

I don't understand why Japanese authors are so fixated on this topic and so consistently wrong. Simply having a foreskin is not in any way the same as having phimosis. If you never learned to pull back your foreskin properly to wash everything in there, well... First, ew. That nasty. You nasty. Second, start doing it! Act like a big boy and clean your bits. Unless they have an extremely specific fetish, ain't nobody wanna get with your unhygienic junk.

I assume that it's mostly a case of different word nuance in translation.

What we medically refer to only as phimosis might be just a general symptomatic word for "Unusually tight foreskin" for them, I'm no authority on this though.

I can however, say with certainty that the basic symptom of phimosis isn't an all or nothing deal. Plenty of people who don't have medical phimosis can still have a level of foreskin tightness that can interfere with sex. I, myself, can not comfortably pull or keep my foreskin back while erect. Presumably, reading this chapter, something along those lines is what the author is referring to.

Why does every manga have a character with phimosis in it? I believe that actual medical phimosis (where it is difficult to peel back the foreskin) is not any more common in Japan than anywhere else (it is rare). It seems like they don't differentiate between "uncircumcised" and actual medical phimosis.

Just having foreskin isn't a medical condition.
