I remember my mom having to devote so much time to caring for my grandparents before they finally got a place at a nursing home. My uncle barely did anything and used work as an excuse even though him and my mom worked the same amount of hours

They explain it in the next chapter but basically he's asking how she was able to afford to pay for college. Narumi doesn't pick up on that fact and replies that she took the train which essentially answers the question for him that her parents paid for her college tuition and that she grew up somewhat sheltered.

Lucky girl never had to deal with student loans. I can also, in a slightly obtuse way, totally relate to the old people being suddenly sharp during assessments bit. My mom would complain of certain joint pains when I went back to visit her. So I took her to a specialist once and when asked the “on a scale of 1 to 10, how painful is this” question, she would suddenly grow balls and say “2 or 3”. I told the Doc it was definitely a 9 or 10 judging by the way she bitched to me.
