A disease that remakes you on a genetic level. Without some sort of recording of the transformation, there's basically no proof that the individual in question is the same person they're claiming to be. They really would come off as a deranged person.

Well, you're not wrong but I would assume there would be a lot of people getting the disease though, then suddenly a rise in people saying they're this person and such; just doesn't feel right.

That would depend on what kind of disease it is. It's not likely viral, since the cause is unknown, so it's likely more along the lines of cancer where the main issue is that the body just isn't functioning correctly anymore.
