My assumption is that they are all unconscious, and are hooked up to a neural network at a facility for the rehabilitation individuals and families. Presumably the initial accident and subsequent accident is a method that the psychologists use to allow for the excuse of the mind to forget their past trauma.

The other individuals in the world are likely AI constructs that flesh everything out.

It's an interesting, quirky comic. I like it so far.
