Yeah that's exactly it, he is overthinking it. In the moment he thinks he needs to go further and further to satisfy her but he's so afraid of actually hurting her that he doesn't know what to do.

And that's tough. S&M is not for the weak minded. You have to communicate with your partner extremely well to understand what they want and what you want. Hopefully these two figure that out and work things out.

Yep, it's all fun and games till you arrive to the "serious" shit. His reaction is pretty normal. You really need to have it in you to satisfy what she really wants. Still, he shouldn't have the word "normal" slipped out. Thought she needs to also understand that there are VERY few people out there who can match her "freak". Like the other S&M romance manga where the male lead is more or less a mind reader. This one is more realistic.
