The problem with documenting everything is that the people abusing their positions may be careful to keep it undocumented, careful to not leave a paper trail. I worked in a toxic workplace or rather, I stepped on a ticking time bomb of a position due to workplace politics that existed before I even got there. The person above me and my trainer were careful to keep it off the books. Thinking back, I should've tried to form better relationships with the people there, ask them to help me rather than focus on doing better at my job, but I didn't know it then.
