Forgot Guilty Crown had the taking a sword out of the chest stuff. I think the first time I saw that from a non-eroge was from the Shining series. It's not that uncommon. That area on a woman's chest is the stuff of hopes and dreams for boys, after all.
Utena was my first. I've seen it in a few mech animes too, it isn't limited to a woman's bosom.
Though Utena really pinned down the posing and style for the two-person sword draw. Everyone since has been drinking deeply from that cup.
A blade drawn straight from the heart bears a nice bit of visual symbolism. Though when Mazinkaiser does it, it just means someone's about to die violently.

Speaking of mech animes, Voltes Five is actually the earliest implementation of the concept that I know of, though it hadn't fully-formed yet. (It isn't where I FIRST saw it, but it's the earliest implemented example I know.)
