I'm assuming they're referring to the rock paper scissors match between Uruma & Jun and Jun thought there would be no penalty if he lost the matches. Hence he "caused her death" by not partaking in the match resulting in Kaname dead. Although this doesn't make sense due to Jun not knowing that his penalty for losing was Kaname's death.

Wait there's level to this. Kaname was originally bullied by the Deliquent 5, but Uruma tried to help her out. The 5 weren't initially interested in him, but Jun who was always a freak noticed that his sister was falling in love. So out of the desperation born from his love, he had the Delinquent 5 change targets to bully Uruma until he went away.
