Why did he not try to hit the fifth/last pitch? she said he would homerun if he tried.

The umpire (the guy who stand behind the catcher, can also understand as a referee in baseball) have concluded the game after the fourth strike by saying "Strike, Strike out, Batter out".
More specific about the rule of baseball, three strikes make an out and Yota have say that the game "ends with with one out". The first strike is ball so it's not count as a strike, but the thrower has scored 3 consecutive strike make the game ends and there are no fifth strike. That's why he freak out, it's a little bit confusing if you have a lack of knowledge in this field, sorry for not explain clearly in the chapter in the first place.

you are the translator, not the mangaka. Not your responsibility to explain stuff like this in the chapter (though I suppose nothing stops you either :p).

Your explanation did not explain why there was no fifth pitch anyway, since all you explained was the rules of the game. But we knew there would be a fifth one because the omniscient girl [seemingly] said there would be.

Seems you have overrated her power. Throughout the story, she didn't make 100% accurate prediction anyway, half way through vol. 2 and you will have the answer for that. I'm working on the final chapter of vol. 1 for this weekend right now. Next week we'll have vol. 2

She is quite literally omniscient.
It has allowed her to know it rained hours before it happened (ok, that is doable for humans), what horses will finish in what order (without any accessible knowledge of the actual area or horses, only of "when" and the concept of "horse racing". So at the very least it is not a laplace-demon kind of omniscience), and the exact actions people will take (and the results they produce) hours if not day before they do (even when it is a reaction to a reaction to her actions!), and that the world will end in 30 days. And more. You can't do all this if your prediction is not 100% correct.
