I kinda wonder how much Yotsuba knows about what Eito did to Uichi (she knows the naked photo incident and gets the bastard’s phone destroyed for it, and the beatings given Eito posted it on his own account for fun), does she know the strangling incident.

If Uichi told her about the strangling incident it says a lot of how bad Rei betrayed him to the point he rather talk about a near death experience at the hands of a peer than ever talk about Rei again…

The mastermind shouldn't be the brother. If not getting the surgery kills Yotsuha and he went back in time to kill Uichi because of it, it would make sense. However it contradicts the point of him saving Yotsuha as he would surely want Yotsuha to be happy and that is only possible with Uichi being happy, unless he is not aware of that. I have a feeling the mother didn't really die in an accident after all and him making her promise not to kill is him preventing her from going down the same path he did.

Damn can’t believe they haven’t found a cure for unspecified anime girl disease that killed Nagisa back in 2004.

We already know what it is, though. She's always been frail...then her mom forced her to go through hellish training to the point that her body practically broke down and she developed heart issues. She has a treatment that would help her but doesn't want to do it because the downtime. Read the damn story.
