It's exactly because he started before the looping. As someone else mentioned the obsession trait runs in the family so it's possible that he was really just jealous that yotsuha didn't like him and wanted uichi to die. But as he put his plans in motion she somehow seems to appear and save him whenever something happens so he could suspect her of having some kinda power and is playing the long game like she said about them maybe trying to wait till the watch breaks. Total speculation tho

It depends entirely on when he and their mom became aware of her. There is still a potential scenario where they were aware of her long before they showed up to adopt her, taking an interest only when the mom wanted to impose her ambitions on her.

It is weird that only Yotsuba was given up for adoption, while the mother kept her brother around. Maybe initially the brother was subjected to the same expectations, with Yotsuba brought in as a spare when those expectations were not being met. During all that time, it is entirely possible that the brother wanted to make her miserable (for the sake of argument, because the brother wanted to be an only child again and she would get in the way of that), but knowing Uichi was there to cheer her up and be a friend to her could have motivated him to shift his attention to try to break Uichi through various pawns, thinking that without Uichi Yotsuba would have nobody else left.

This theory would hinge on the brother being at least as obsessive as their mom and it doesn't quite line up with things persisting even after the mom's death (though that obsession could have shifted to outright hatred, now blaming Yotsuba for the death and denying him ever being able to go back to being their mom's only child), but I don't think this theory is contradicted by anything we have read so far. It's just a theory held up by duct tape and string.

It would make sense for the brother or someone he knew personally to be the mastermind; the obsession trait runs in the blood there. For good and bad...

A little thing could absolutely be the (stupid) reason to start this ridiculous cycle...
The brother doesn't know him though. What's the motive? Secretly wanting to crush his sister while also helping her? It'd be a pretty wild stretch.

When she dies wouldn't uichi just regress to how he was before he met her?
Hence why when we were exploring the past she said she's on two time limits and she's on a rush to finish everything.
To ensure Uichi would be fine on his own while she goes for the surgery.

Now we just need to hand Yotsuba a glock and point her in the right direction, and the happy ending will be achieved.
